Gut Microbiome & Probiotics Interaction:

Bacterial cells retain within the human gastrointestinal tract as a host and their genes are encoded by the bacterial resident. These microbes which are associated inside the human digestive- tract are called as a gut microbiome. These are mainly involved in human metabolism, Nutrition, physiology, and immune function. Gut microbes undergo many metabolic functions that humans are unable to do and produce ample varieties of nutrients, proteins, polysaccharides which will cause either good to the human as probiotics or causes harm to developing diseases like Bacterial Infection, Obesity, diabetes, atopy, Inflammatory bowel disease, tumors etc. Microbes will develop a highly- uninterruptable chip (Genome), but some researchers find out using metagenomic and metaproteomic studies, for example, 16S rRNA profiling.

There are lots of questions in our mind like do probiotics help the gut to promote the interactions. Here we go, there are two possibilities one is by taking the probiotics will help our digestive system and eliminate the unwanted bacteria. Second, is the will mix with our human bacteria and will cause reverse action towards our intestine. But until now no one sure about the causing factor for promoting the intestinal diseases either aerobic bacteria or anaerobic bacteria.

Gastric acid, gastric juice or stomach acid, is a digestive fluid formed in the stomach and is composed of HCL, KCL, NaCl which are playing a key role in the digestion of Proteins. Gastric Acid mainly produces from the inner lining of the stomach cells. The Other cells in the stomach will produce sodium bicarbonate to balance the acidity in the stomach. The excess secretion of acid leads to the ulcers, gastric acid is mainly regulated by the autonomous Nervous system and several hormones may produce Gastrin which will allow producing too much acid in the stomach, which turns to cause duodenal ulcers, peptic ulcers, pancreatic ulcers, stomach ulcers, intestinal ulcers. The Main production of HCl in the stomach is to protect against the bacterial overgrowth in the gut. Achlorhydria and hypochlorhydria are common digestive problems. These causes several problems like Heartburn, Bloating, Indigestion, gallstones, asthma, impaired Acid secretion, Diarrhea, Mineral deficiency, Iron deficiency. According to the British Society of Gastroenterology (BSG), every five out of ten is suffering from Acid-related diseases.

Gastrointestinal Bleeding:

Gastrointestinal Bleeding is also known as Gastrointestinal Haemorrhage. It predominantly takes place in the intestinal region (GI Tract) i.e from mouth to anus, like vomiting red blood, vomiting black blood and stools are dark red and dark black etc. sometimes bleeding flows inside the tissues by rupturing the inner walls of the tissue without any symptom, the major bleeding called upper Gastrointestinal bleeding. These cause the peptic ulcers, variceal bleeding by liver cirrhosis, Intestinal Cancer. Upper Gastrointestinal endoscopy is the foremost step in the Gastrointestinal bleeding (GI) for therapeutic purpose if the consequence rules out any upper GI bleeding, then colonoscopy is preferred for convincing that the bleeding is in the lower GI tract. Microscopic levels of bleeding can lead to anemia over time, and more massive amounts of bleeding can lead to death. Gastrointestinal Bleeding causes many severe and several problems inside the intestine. Upper GI bleeding has been Assessed to represent for up to 20 000 deaths yearly in the United States.

Gastrointestinal Pathology:

Gastrointestinal pathology is the subspecialty in the domain of surgical pathology which deals with the identification and characterization of neoplastic (abnormal and excessive growth of tissue) and non-neoplastic diseases of the digestive tract, Stomach and accessory organs, such as the pancreas and liver.

Digestive diseases influence 1 of every 5 Americans every year – somewhere around 60 million individuals in the United States every year. Digestive diseases represent roughly 50 million doctor visits, almost 25 million endoscopic symptomatic procedures and more than 20 million gastrointestinal specimens directed to pathologists – representing $87 billion in direct medical costs attributable to the gastrointestinal disease. The interest in gastroenterology care and demonstrative techniques keep on expanding, driven by an expanding frequency of gastrointestinal infection and the requirement for colorectal malignant growth screening in the maturing U.S. populace. By 2018, gastroenterologists will perform somewhere around 40 million endoscopic strategies yearly, converting into $40 billion endoscopies and $12 billion related gastrointestinal pathology costs every year in the U.S.

Pancreatology:

The pancreas is one of the masters in the scientific experts of the body that produce hormones exocrine and endocrine to aid the absorption of nourishment. It comprises various kinds of pancreatic cells. The exocrine cells incorporate acinar cells and the endocrine cells are available in bunches of cells called the islets of Langerhans. The Islets of Langerhans work with the assistance of pancreatic alpha, beta, delta and gamma cells. The gallbladder is little sac-molded organ underneath the liver, in which bile is put away after emission by the liver and before discharge into the digestive tract. Genetical screening examines have recognized quality transformations that prompt pancreatic infections and in distinguishing the adjustments in the pancreatic cells influenced by sicknesses.

Gastrointestinal Immunology:

The gastrointestinal tract plays dual roles in human physiology one is digestion and uptake of nutrients and another one is the more difficult task of maintaining immune homeostasis i.e protecting the body from harmful microbes. No other system has this unique architecture like functioning and regulating both the systems.

The gastrointestinal-associated lymphoid tissue (GALT), which is composed of discrete inductive and effectors sites, can differentiate between the harmful and harmless antigens while maintaining the homeostasis. Inductive sites are well organized into specialized gatherings of lymphoid follicles called Peyer's patches, while effector sites are more diffusely Separated. The separation of these sites serves to limit and control the immune responses. In addition to its Unique architecture, the GI tract has specialized immune cells that aid in promoting a tolerogenic response to orally introduced antigens, e.g. subsets of dendritic cells (DCs) and regulatory T-cells, which is produced in many quantities at mucosal surfaces, and it promotes and nurtures an anti-inflammatory environment by neutralizing immune stimulatory antigens.

Gastrointestinal Nutrition and Dietetics:

Gastrointestinal Nutrition play’s a huge role in the human body and it regulates and systematizes every organ in the body, the gut interacts and produce the immunity. Likewise, the nutritional requirements of the body are considered and here too the gut plays a chief role. A poorly functioning gut can not only be uncomfortable it can also have a serious impact on the physiological function as well as psychologically. Expanded admissions of vitality, protein, nutrients, minerals, and electrolytes are regularly required to supplant supplements lost because of hindered stomach related and absorptive limit. Consistency, dinner recurrence, and different attributes of the eating routine might be adjusted to meet the patient's requirements. Medical Nutrition Therapy (MNT) for all patients with ailments of the digestive organs must be individualized. The essential accentuation in the dietary administration is the decrease of carb sustenance’s that are probably malabsorption and matured, including vegetables, solvent fiber, safe starches, and straightforward sugars, for example, fructose and liquor sugars.

Obesity and Diabetes:

Obesity and Diabetes are the current escalating problem in the World. There is a huge relation between obesity and diabetes. These are the chronic disorders and most affected diseases worldwide. Body Mass Index (BMI) is the driving force between obesity and Diabetes. BMI has a strong relationship with diabetes and insulin resistance. In obese individuals having large amounts of fats, non-steroid fatty acids, Glycerol’s, proinflammatory Markers these substances develop the insulin resistance. The main cause of diabetes is ?-islet cells of the pancreas are impaired, causing a lack of control on blood glucose level. Weight gain and body mass central formulation for rising diabetes.

The Centres for Disease Control and Prevention report that 32% white and 53% of black women are obese. Women with a body mass index (BMI) of 30 kg/m2 have a 28 times greater risk of developing diabetes than do women of normal weight. The risk of diabetes is 93 times greater if the BMI is 35 kg/m2. The presence of diabetes can increase a woman's risk of heart disease 2-fold.

Gastrointestinal Endocrinology:

Gastrointestinal endocrinology has grown explosively during the last quarter of this century, there are three hormones (secretin, gastrin, and cholecystokinin) were identified and by authorities in the field considered enough to explain the entire hormonal regulation of digestion. That was some underestimation. Today gut is known to express in excess of 20 distinctive hormonal/administrative peptide systems with different controlling system and regulations in the entire digestive system. Their widespread cellular occurrence, gene expression cascades, secretory mechanisms, receptors, and receptor binding, as well as normal and pathophysiological effects, are now also well-known owing to the marked progress in basic sciences and biochemical technologies (immune and peptides chemistry, molecular and cell biology). Thus, the gut is now recognized as the largest endocrine organ of the body and a substantial part of the gastroenterological research over the latest decades has been devoted to gut hormones. Hence gastrointestinal endocrinology has been a major research area in this country in the past 25 years.
